  1. THe physician or doctor is the one on the left, supervising the operation and applying the ether on the patients face. The man in black holding the saw is the butcher. Physicians consider it beneath their dignity to do surgical work so they employed butchers. THus surgeons evolved from butchers and up till today, liked to be addressed as Mr X rather than Dr X. The divide continues with a College of Physicians and a saperate College of Surgeons for the modern doctors.
