This video is like watching an episode of ER, only this is real life, not staged. The patient was brought it by helicopter after taking a shotgun blast to the face. The wound was self inflicted, but I’m not entirely sure whether this was an accidental discharge, or an attempted suicide. I tend to lean towards the former. Talk about a shitty day if you blast off your own face with your own shotgun.
As you can gather from the conversation between the surgeons and assisting nurses, the patient is alive and conscious. He was lucky that the bullet only took his face off but left his brain untouched. No major neck vessel was ruptured either so brain kept getting its necessary supply of blood and kept functioning normally. That part is a good news.
The bad news is that an entire bone structure below his orbital floors was destroyed. The blast obliterated the mandible (lower jaw bone), maxilla (bone forming upper jaw), palate (the roof of the mouth) and both zygomas (zygomatic bones, aka bones that form the cheeks). Soft tissue of the face as well as the airways were also severely damaged.
Two thirds of the patient’s tongue were lost to the blast, but the remainder helped the surgeons to locate the trachea and apply the tracheal intubation to let the air in and out of the lungs. Patient also had to undergo several plastic surgeries. All I can say after watching this video is – careful where you point your shotgun… 10 minutes long video of the surgical procedure is below. That shotgun pic is what we call a “flincher.” When someone wants to commit suicide and doesn’t have the ability to follow through. They flinch and pull it forward blowing their face off. They usually survive, but live the rest of their lives on a machine.
The worst part about these patients is that they are writhing in pain and can’t say what’s wrong. Their face is gone so we tube them and med them and let the OR’s do the rest!

Cambodia has a very, very long history of bloodshed and violence. You can go through history books and read up on any part of Cambodian past and all you will find will be violence. The country as it is today is not different from the way it has always been. With its stellar corruption record and a complete joke of a police force, a value of human lives is in the neighbourhood of $20. Gun ownership is unregulated and uncontrolled, with handguns sold in the streets in broad daylight and thousands upon thousands of military grade weapons and explosives still floating around since the Khmer Rouge rule. Killing someone in Cambodia is as easy as pulling out that handgun you have and shooting them in the head in the street. In 9 out of 10 cases, the body will be eaten by stray dogs and nobody will bother to investigate into it. In a rare case that it’s someone associated with the powerful Cambodian elite, the heads will be chopped. Cambodian prime minister Hun Sen is a totalitarian dictator second in the number of people he had removed from his path only to Hitler or Stalin. Cambodia just rocks like that.
What kind of people do you think a society like this produces? I was gonna say – ask any of the foreigners who were brutally murdered in Cambodia, but they can no longer speak. Some were murdered for 2,000 Cambodian Riels (equivalent of 50 US cents) – just ask the family of Australian John Edward Thompson, others were beaten to death and as their body was laying there lifeless, the perpetrators stole all of their belongings, including their clothes letting them die in a ditch naked – just ask the family of Canadian Jiri Zivny who volunteered in Cambodia as an Aid Worker. The list of foreigners violently murdered in Cambodia just goes on.
Cambodia is also the rape capital of the world with men freely raping everyone from small children to elderly women without any form of recourse. Would it surprise anyone that Cambodia has some of the world’s highest rates of acid attacks per capita in the world? Since its induction in 2006, Cambodian Acid Survivors Charity registered 270 acid attack victims (CASC only registers victims who seek treatment and support through them, hence actual number of victims is much higher). 10 New cases were added to the total in the first three months of 2010 and by June of the same year – just before pictures of Sokreun Mean, the acid attack victim from the gallery below were taken – the number of acid attack victims grew by 17. There is basically one acid attack somewhere in Cambodia every few days.
These are no hoax acid attacks. Unlike the recent case of Bethany Storro from Vancouver, Canada, the attacks in Cambodia are very much real and victims end up severely scarred. Sokreun Mean lost her eyes and had her face severely disfigured after estranged wife of her ex-husband threw large quantity of acid on her. Sokreun Mean has been operated on more than 20 times but remains vastly disfigured and completely blind.

Isabelle Dinoire is a French woman who was mauled by a Labrador dog after she had overdosed on sleeping pills in May of 2005. While she was passed out, Tania – the dog Isabelle Dinoire loved so much came to her and started scratching and chewing on her face. There are speculations that the dog got frantic after seeing her master unresponsive and was possibly trying to wake her. After she was mauled, Isabelle Dinoire was left without lips, with part of her nose, chin and cheeks missing. She was scheduled to undergo face transplant operation which took place on November 27, 2005 at the Centre Hospitalier Universitaire Nord in Amiens, France. Isabelle Dinoire is the first person ever to have undergone a partial face transplant and these are the photos demonstrating the tragedy and the breakthrough in surgical medicine.
Isabelle Dinoire came from Valenciennes in northern France. She had no memory of the dog attack. She overdosed on sleeping pills and spent quality time floating somewhere in the fifth dimension while her face was being eaten by a Labrador dog. After she woke up, she could see the floor covered in blood. She reached for a cigarette box, pulled one out and went to light it up when she realized she had no lips and the blood on the floor was her own.
There are controversial reports as to whether Isabelle Dinoire attempted a suicide when she overdosed on sleeping pills. Some sources, including Isabelle Dinoire’s daughter as well as orthopedic surgeon Olivier Jardé from the hospital in Amiens where the face transplant was performed claim that she did attempt suicide. Professor Jean-Michel Dubernard from Amiens who assisted with her face transplant operation however says that it was an accidental overdose, not a suicide. The identity of the donor is hidden, however there are also speculations that the donor died by killing herself (hanging).
Isabelle Dinoire later allegedly confirmed suicide attempt to the British The Sunday Times in a phone interview. She is also claimed to have said that she felt connection with the donor because they both were suicidal.
Transplants of internal body organs had been performed by surgeons for years before Isabelle Dinoire’s operation. We have all heard about heart transplants, kidney transplants or liver transplants, but such thing as face transplant had not been performed until 2005 when Isabelle Dinoire was mauled by her dog. Part of the reason is that people are very sensitive about their faces as your face is something that identifies you. It’s the sacred part of your identity.
It is also understandable that transplants of internal organs are life-saving operations, whereas face transplant isn’t. But as it is with transplants of internal organs, face transplants also require a donor, which ultimately results in change of facial features of the receiver. After you have receive a face transplant, each time you look in the mirror, you will be literally looking at a face of that other person.
Professor Bernard Devauchelle who performed face transplant operation on Isabelle Dinoire with assistance from Professor Jean-Michel Dubernard also implanted bone marrow cells into the patient in order to avoid rejection of new tissue by the system. Isabelle Dinoire was also served immunosuppressants in order to minimize tissue rejections.
Surprisingly, despite her immunity striving hard to fight with what it thought was an alien tissue, Isabelle Dinoire soon regained sensation in her new face. Physio classes helped her reactivate facial muscles and despite hardship from the beginning, she’s getting used to her new “identity” so to speak.
The Labrador Dog that mauled her had to be euthanized.
